Needed documents for a new ID

• Completed application form 01 of department of Individual registration should certify by Grama Niladari.
• Applicant should be 16 years old.
• Should paste Rs.13.00 value stamps.Should attach original and certified true copy of birth certificate by Grama Niladari.
• If applicant is old more than 50 should present a letter certify by GN giving reasons to delay with an oath statement.
• Need 02 more extra photos other than attached 03 photos in the application.
• If applicant is more than 50 and no birth certificate should present predicted age certificate or certificate registered as a post dated birth.
• If applicant is less than 50 and no birth certificate should present vacant result sheet with children’s birth certificates.
• If needed all documents are completed application form 13 of department of Individual registration should noted and should presentto AO/AG for certification.

Needed documents to receive a new ID behalf of lost ID

• Police report (should not exceed 01 year).
• Should paste Rs.15.00 value stamps to the application form.
• Should attach original and copy of birth certificate.
• If applicant is more than 50 and no birth certificate should present vacant result sheet with children’s birth certificates.
• If applicant is less than 50 and no birth certificate should present predicted age certificate or certificate registered as a post dated birth.
• Need lost ID number and written proof to used ID. (eg: passport, driving license or bank accounts  certified photo copies).
• Need 02 more extra photos other than attached 03 photos in the application.
• Need service letter received within 03 months to include post holding.
• If using name after marriage need marriage certificate.
• If needed all documents are completed application form 7 and 13 of department of Individual registration should noted and shouldresent to AO/AG and AAG for certification.

Needed documents to receive an amended ID

• Completed application form 08 of department of Individual registration should certify by Grama Niladari.
• Need 02 more extra photos other than attached 03 photos in the application.
• Should paste Rs.15.00 value stamps to the application form.
• Old ID.
• Need birth certificate if old ID is not clear.
• If applicant is more than 50 and no birth certificate should present vacant result sheet with children’s birth certificates.
• If applicant is less than 50 and no birth certificate should present predicted age certificate or certificate registered as a post dated birth.
• If using name after marriage need marriage certificate.
• If needed all documents are completed application form 8 and 13 of department of Individual registration should noted and shouldpresent to AO/AG and AAG for certification.

Receiving a public fund (Asthma, Wheezing, Heart diseases, Fit etc.)

• Requesting letter.
• Model form signed by applicant (SS/M/A 01).
• GN report (SS/M/A 02).
• Medical certificate if suffering from any disease.
• Approval of Social Services Officer.
• Approval of AG/AAG.Noted in the list.
• Approval of Social Services Director.
• If approved including to paying list if there are funds.
• If no funds available receiving from department of social services.
• Issuing written card if AG has signed.

Income Certificates

• License issues on 8.45 a.m. to 3.00 p.m. during weekdays.
• Charge the license fee 1.27% of income.
